Payment and Forms

For your convenience, we offer the option to download and complete your patient forms and privacy policy before your visit—simply click the links below to fill them out and bring them with you to save time during your appointment. Don’t forget to check your dental insurance benefits before the end of the year, as many plans reset on December 31st and any unused benefits may be lost. If you have dental insurance, we accept most major plans and file claims as a courtesy. We also provide a 7% accounting adjustment when treatment is paid in full by cash or check on the day services are rendered. Additionally, we accept all major credit cards, including Visa, MasterCard, American Express, Discover, and offer Care Credit for flexible financing options.
